It uses a 16-position footprint with only 12 pins populated.Positions 4, 5, 12 and 13 are empty, so the outline drawing, suggested pad layout and schematic all show 12 pins, six per side on a 1.27 mm Pitch.
It is an SMD(Low height) part with a maximum height of 2.50 mm.Operating Temperature is 0 to +70°C, with storage rated from -40 to +85°C. Vt is 1500 V (RMS), verified by a Hipot test at 1500 VAC, 1 mA for 60 s.
